Date-based tracking answers one question — is the paper current — and organisations quietly treat it as answering another: does the person still know it. The gap between the two is where incidents live: the forklift certification with eighteen months left and a holder who hasn’t lifted in a year; the first-aid card valid until 2027 held by someone who couldn’t find the AED. What about certifications with practical assessments?

How do we price the single-holder risk the matrix shows?
